End User License Agreement
The developers give permission for you, a health care provider knowledgeable in cardiac medicine (You), to use the Prediction of survival in cardiac failure web application. The application is a calculator of projected survival at baseline and after interventions for patients with cardiac failure. The developers allow You to use application on the following conditions:
You acknowledge and agree that the intended audience for the application is health care professionals knowledgeable in cardiac medicine. If You are not a health care provider, You may not use application.
The developers reserve the right to modify or make improvements in the application at any time without notice.
You agree that if you provide feedback to the Developers, that the Developers are permitted to use any information provided by You in making changes to the application.
You acknowledge and agree as a condition of using application, that the information contained in application is intended as a supplement to, and not a substitute for, Your knowledge, expertise, skill and professional judgment. The developers make no representations or warranty on the accuracy, validity, completeness or availability of information or results obtained from the application. Information provided by the application should not be construed to indicate that any particular outcome, treatment or treatment combination is safe, appropriate or effective in any given patient. The model used by application was constructed from past data trends collected from research studies and may not be predictive of a given patient's risks. You acknowledge that the application is solely a health care provider information resource and is made available as a research courtesy "AS IS," without obligation to provide accompanying services or support.
The authors of the accompanying paper own the intellectual property in application. Nothing contained in this Agreement shall be construed as conferring any right to use in advertising, publicity or other promotional activities any name, trade name, trademark, or other designation of UW or Developers without express written permission.
The developers expressly disclaim any and all warranties regarding the application, whether express or implied, including but not limited to warranties pertaining to merchantability or fitness for a particular purpose. Developers will not be liable directly or indirectly for any property damage, personal injury, loss of use, interruption of business, loss of profits, or other special, incidental or consequential damages, however caused, whether for breach of warranty, contract, tort (including negligence), strict liability or otherwise. (Some jurisdictions do not allow the exclusion or limitation of incidental or consequential damages, so this exclusion and limitation may not apply to You).
